Don't want to apply online?
The Social Security Administration encourages everyone to apply online, but the online application process isn't right for everyone. Depending on your situation, here are needed forms for the paper option. You can fax in or drop off these forms at your local Social Security office location..
- If you are already enrolled in Medicare Part A and are ready to sign up for Part B, you don't have to apply online - there's a form for that :)
- If you are already enrolled in Medicare Part A, and are enrolling in Part B, you may also need an Employer Verification form as well, if you are over the age of 65 and have had benefits provided by your employer..
